Confidentiality  

What it means.

Confidentiality means that the information you share with me stays private.  With only a few exceptions, I do not reveal any information about my clients without specific permission.  You can feel confident knowing that what you tell me stays between us.  Please read this example of a Consent form for more detailed information about confidentiality.  


Electronic security and privacy  

Electronic communication such as email, text messages, and instant messaging are very convenient and can be very helpful. However, it is important to know that they are not as secure as talking on the phone or using regular mail.  This is because copies of each email, text, and instant message get saved by the company that provides that service.  


Example Forms

Client Information Form - this form provides information about you that helps me give you excellent service.  

Consent - this form outlines my policies, describes the nature of my services, explains confidentiality and privacy, and is our way of documenting that we have chosen to work together.

Release of Information - if you complete and sign this form, it allows me to share information about our work together with the person or organization you name on the form. 

Electronic Communication Consent - if you choose to communicate with me electronically, this form outlines the limitations of electronic communication and your signature means that you agree to these terms.
